Att 2-2 The Certificate of related Business qualification: the Certificate of Design in related business, the manufacturing Certificate, ISO9000 Certificate, API Certificate, ASME Certificate, etc..   3.    Performance Information and Contract Execution (Incl. delivery and technical support) 3.1  Please state lead time and normal delivery period of your goods as well as the shortest delivery period in an emergency. Att4-2  The financial statements including Balance Sheet, Cash Flow Statement and Profit Statement for the last three consecutive fiscal years (2010-2012), which must bear the audit report (copy) issued by the qualified accounting firm Att4-3  Written Commitment indicating Letter of Guarantee or bid security will be offered at the stage of bidding activities and Performance Guarantee or Standby Letter of Credit for the fulfillment of the contractual obligations according the Buyer’s requirement.   5.    Reference Performance of Supplier Please provide the following information (Att 5-1): Att5-1  The reference list of the same or similar products (as mentioned in the above Main Business Scope) applied in large petrochemical projects in China (i.e. ethylene project with the capacity over 600 KTA and refinery project with the capacity over 8000 KTA) over the past five years. Att5-2  The reference list of the same or similar products applied in large projects that are under going by CNPC, Sinopec and CNOOC over the past three years. Att5-3 The reference list of the same or similar products applied in other petrochemical enterprises under the similar operational status over the past five years. The format above is for your reference.   6.    Quality Management System  Att6-1  Please provide the brief introduction of your company’s quality management system and relevant certificates (such as ISO9000 certification, industrial standards certificates) supporting your ability in this regard. The brief introduction shall include but not limited to the following content: (a)  The manufacturing and inspection criteria of your company; whether your company have the quality criteria for your own products; (a)  Whether your company review the sub-supplier’s supply ability; if so, please state the frequency of inspection, whether your company archive the written results, whether your company have formal receiving and inspection system. (b)  What kind of quality inspection equipment your company has; (c)  Main items and methods for ex-work test or inspection; (d)  Time limit of quality or performance guarantee; (e)  After-sales guarantee measures for your manufactured equipment; (f)   Whether your Company actively adopted Six Sigma initiatives within your Management model. Att6-2 Manufacturing, Inspection and Capacity information need to be provided as follows: (a)  The main status and capacity of the processing equipment, inspection and test equipment. (b)  Details of Manufacturing capability and after-sale service within China; (c)  Details of Representative Office or exclusive agent in China. Att6-3  The quality and after-sale service information for the relevant products over the past five years.
